    My guess would be never.

    So far as I know, only Massachusetts and Hawaii have provisions about employee health benefits.

      When a bona fide contract or CBA calls for it.

      Scott is correct. The only states where the law requires an employer, regardless of whether they are for-profit or not-for-profit, to provide health insurance are Hawaii and Massachusetts (and not even always there).
      The above answer, whatever it is, assumes that no legally binding and enforceable contract or CBA says otherwise. If it does, then the terms of the contract or CBA apply.
