    Do I have to give a doctors note to disability when I file my claim?

    Are you talking about Ca. SDI? Yes, you would need a doctor's statement that
    you qualify for disability benefits. (That you are disabled.)

        I cannot think of any circumstances in any state when a doctor's certification is not required for disability benefits.
        The above answer, whatever it is, assumes that no legally binding and enforceable contract or CBA says otherwise. If it does, then the terms of the contract or CBA apply.

          Agree, they aren't going to take your word for it that you are disabled. You
          always need a doctor's statement that you have a disability.
