my accountant had a question about the insurance bill, she did not come to me (HR manager), in stead, she went to directly to the employee to ask him if he had disability and accident insurance, did she cross the line ?

On the basis of what you have posted, no. I don't see any reason why she shouldn't ask the employee directly about his insurance.
If you want to provide additional information, such as who you are, what kind of insurance it was, what your accountant's question was, and why you think she should have come to you instead, we can revist the question and see if the answer remains the same.The above answer, whatever it is, assumes that no legally binding and enforceable contract or CBA says otherwise. If it does, then the terms of the contract or CBA apply.
