    Our intake counter clerks have found that the top four errors in the filing of documents are:

    Failure to attach an extra copy of the documents filed in the cases of Senior Judges Jaime Pieras, Jr. and Raymond L. Acosta.

    Filing bankruptcy motions before the District Court instead of the Bankruptcy Court.

    Filing documents without the attorney's original signature.

    Filing documents without the complete case number: category (civil/criminal/miscellaneous), case number and initials of the presiding judge.

    Also, there has been times when documents have been filed without the necessary attachments; in a copy rather than the original; without payment of filing fees; or in the wrong court units such as documents for the U.S. Pretrial Services Office or the U.S. Probation Office.

    You may use this list as a quick reminder when members of your staff or commercial messenger services file your documents with the Court, or even prepare a checklist to attach to your pleadings to be filed. Avoiding these common mistakes will expedite the processing of your pleadings.
