    Default Getting started New York

I live in Ct but work in NY. May be needing spine surgery, (fusion at S1/L5/L4 and laminectomy in few spots) which would keep me out for some time. I am a plumber so my job requires physical exertion. Second issue is, my hours were recently cut back from 40 to 28 per week. This has nothing to do with my injury...which is also not a worker's comp issue. Don't really know if I'm now considered "part time". My questions are...If I go short term disability, do I file in NY? Who do I contact to file? What impact, if any, does my reduced work hours have? Thanks
