Does an employer have to give an employee a separation notice in the state of GA, regardless of the reason? i.e. I quit. And if so, how long do they have to get it to you?

In Ga., yes, they are to furnish you with a separation notice. It should be given on the last day of work.

Too often we underestimate the power of a touch, a smile, a kind word, a listening ear, an honest compliment, or the smallest act of caring, all of which have the potential to turn a life around. Leo Buscaglia

Is there a penalty or something if they fail to do so?

If there is, it would be payable to the state, not to the ex-employee.
