If a tenant loses their copy of the lease, is the landlord required to provide them with another?

Depends on the state and the lease. Usually, if there is a situation where the lease is being enforced, a tenant can ask that the landlord show where on the lease the enforcement relates to and demand a copy of the lease. Otherwise, probably not a requirement, and landlord may even charge a reasonable fee for getting a lease copy.
