can my employer force to come in early even if i have doctors apointment that time they want me to be there and give me points for not showing up then i work about 60 hours every week and somtimes moore so now i feel i cant go to therapy for my shoulder any moore

Of course, you cannot be forced , but they can discipline you for absences, even if it is to go to the doctor. I'm not saying I agree with this, but it isn't illegal. Having said that, I believe that arthritis, may be considered a chronic condition which could possibly be a FMLA-qualifying condition.
Do you know if your employer big enough to be subject to FMLA and, if so, have you been there long enough and worked enough hours to be eligible?

Thanks i agree seems wrong at they can do what they want to do not mutch i can but quit there
i think they have fmla since they belong to emerson corp. and they talk alot of ethic they only thing i can say is absent of ethics to there workers

That's big enough. If you have been employed there long enough, and have worked enough hours, I recommend you speak with HR about getting FMLA paperwork for your doctor to complete. I'm not saying this will turn out to be an FMLA-qualifying condition, but you have nothing to lose by trying.
