I couldn't find anything on Illinois parenting guidelines.. however, here is a link for Indiana's parenting guidelines and you could develop ideas using it because it is RATHER good.

Well at this point in time, you probably just need to get a form to seek modification to include or increase visitation.
You also may want to visit a legal forms site and look at the sample or example forms for child custody-visitation in IL
