I gave my 2 weeks notice and request my regular hrs, OT hrs of 17 hrs and my 2 week paid vacation, be on my final paycheck. I received my final paycheck w/ my regular salary pay. No OT hrs and my 2 wks vacation pay was rated at the pay when I first started with this office. I have since had several raises and promotions. My regular salary was agreed on at a 34 hr. work week and was never changed. Since July, I accured 17 hrs. OT and the Head Dr. said, any hrs. wk will be owed to you, but when requested he did not pay. What grounds do I have with the vacation being at a old pay rate and OT not being paid.?

Your former employer is required to pay you all overtime owed to you. You can learn more about filing a claim in for unpaid wages in Arizona by looking at the following website: http://www.ica.state.az.us/Labor/wagefaq.htm

As far as vacation is concerned, there is no requirement that Arizona employers pay out vacation at all. So, the fact that you are receiving pay for your vacation is more than the law requires.

With that in mind, do you have the vacation policy in writing? Does it specify the way vacation is calculated? If it does and the amount you were paid is in conflict with that policy, you may be able to recover the additional monies.

Thank you so much for your reply.
I do have my benefit pack which it does state for vacation pay if not used by the end of the year the Dr. will cash it out to you so it is not lost, also if leaving with a 2 week notice (which I did) any vacation and sick time not taken will be on final paycheck. However, it does say to be paid at current rates.

You're very welcome. Please let me know how it goes with the payment of overtime. Your employer is definitely obligated to pay you the overtime.
