This employer has insured you with the Employment Security Commission of North Carolina against total loss of pay through lack of work. Workers who are unemployed for other reasons or who refuse suitable work are subject to a disqualification as provided by law.

If you lose your job, you should report at once to the nearest Employment Security Office or point of service. There you will be helped to find other work and assisted in filing your claim for unemployment insurance benefits at no cost to you.

While you are an employee of this establishment, if during any payroll week you do not work at all or you work less than the equivalent of three customary scheduled full-time days, you may be eligible for unemployment insurance benefits. If this happens, your employer will prepare a Benefit Claim for Attached Worker and mail this claim form to Raleigh for processing.
