I am really sorry for asking these questions, I started to look through
past posts and my head is spinning as we are trying to get this done
before we leave on our honeymoon tomorrow!
1. We live in Virginia, can we file all three together at Vermont
Service Center in the same envelope?
2. Do the 3 years tax returns need to be notorized, or just the last
page of I-864?
3. Where do we get the "signature card"?

1. To my knowledge, the only people who file AOS/EAD at Vermont are
people in Maryland.
2. We only got the last page of the I-864 notarized. Never have
notarized the tax transcripts.
3. I heard that the signature card is no longer required -- but again,
that's just as far as I know.

No. You file in your district office for the one covering the Virginia
area you live in, i.e. Norfork, Arlington. VSC is only for those living
in the jurisdiction of Baltimore, Maryland's district office.

Mark only filed for AOS and EAD, so can't answer about AP...
1) We live in Richmond so we sent Mark's applications to the Norfolk
suboffice. We mailed them in one envelope, return receipt, with all
accompanying documentation and three separate checks ($225 for AOS,
$120 for EAD and $50 for fingerprinting). I believe you send your
applications to Arlington; the AOS/EAD packet should tell you exactly
where to send them.
2) We had our I-864 notarized and sent official IRS tax transcripts for
the past 3 tax years, which I ordered over the phone.
3) The signature card was included in the AOS/EAD packet we
received. It was a smallish piece of paper with a box in which
Mark signed his name.
