I'm 8 months pregnant and due at the end of January. I have been working for a temp agency so I don't have paid maternity leave and will not be having a paycheck while on leave. I have a couple questions about the nj state disability. My boss is open for me to leave a month early, beginning of january for maternity leave however, since I won't be getting paid for this, i can't afford it. I know the state allows for disability 4 weeks prior however, does my doctor have to deem me as being disable before i am eligible? I rather work up until the my due date so i can get paid my full amount and save more money. Also, when I am eligible for disability do I just print the form online for the doctor and my employer to fill it out? or do the doctor provide the forms? What is the waiting period before I actually will be collecting a check? I like to be prepared and want to know how to plan. I would like to save but with a new baby and not collecting a normal paycheck is hard to say what will come up and I want to be prepared.

Yes you do have to be deemed disabled by a phsyician before you can collect disability.

You can print out the forms online.
