Hello-
I recently found a form online that was a Request for Termination of Employment. Of course, the printer wasn't working that day so I don't have an actual copy and of course, I don't remember where I found it!
It stated "I am requesting a written statement detailing the reasons for the termination of my employment. Please honor this request within _ days of your receipt of this notice. You may send a statement to me at the following address:"
I realize I have all the information it states, but I was hoping to get the actual form again, thinking it may have a state labor department logo or something official on the bottom so it doesn't look like I just typed it up.
Has anybody seen anything like this or know where I could get something official?

I have no idea if PA requires written reasons for terrmination upon request. My state (Maine) does do that and you don't need a form. Just tell us that you want a letter explaining why you were terminated. You know the reason, already, but this will be in a letter that you may not want to show to potential employers.

I have never heard of such a form, and PA is NOT one of the few states that requires a reason for termination on request. I suspect what you may have found is a company-specific form that some company put out on the Web for their employees.
