If I take 8 hours paid time off in a work week or have 8 hour holiday pay and then work 40 hours in the same work week, is my employer required to pay 8 hours time and on half for the eight hours over forty? If time and !/2 is due for that 8 hours, should it be paid on the check for that same pay period?

By law, you get time and a half for hours WORKED over 40 in a work week. Paid time off (holiday, sick, vacation) does not count as work, so 40 work hours and eight holiday hours = 48 hours straight pay, no overtime.

Note that some companies are generous and do pay OT when they are not legally required to do so.
