An employee injured herself on her way to work. Not at work. She has a doctors note which says she cannot work more then 4 hours a day. We have accomidated the request. The issue is that she has been calling in unable to work shifts, she has requested that she work only 3 hours instead of 4 and she has been working only 12 hours a week and we have a minimum requirement of 15 hours a week. We have requested also requested a doctors but she told us she cannot be required to provide one. Are her requests reasonable and can we require a doctors note?

Not hardley. You only have to make an accomodation based on what the doctor reccomends and as long as it doesn't create an undue harship, not the employee and you can require any additional documentation from the employee's provider to assist in making your decision.
