You may file a proof of claim in person or by mail. A claim form may be obtained from the Clerk’s Office at any location, on our web site at www.insb.uscourts.gov or in certain cases a proof of claim form is located on the back of the Notice of 341 Meeting. If you request a claim by mail, you must include a self-addressed stamped envelope.



You will need to submit an original and one copy of the proof of claim. If you wish to have a file marked copy returned to you an extra copy must be submitted with a self-addressed stamped envelope of sufficient size.
