An interesting question. If you're actually thinking of doing
something like that, you need to consult a lawyer. Medical
malpractice litigation is definitely not a do-it-yourself job.

If you're asking out of curiosity...

I can see one practical problem: the defendant will want your medical
records as part of the discovery process. After all, they are
entitled to know the evidence on which you are claiming that you were
harmed.

For that matter, I expect the defendant will need to be able to look
you up in hiser records to see what, if anything, they did (or didn't
do) to you.

So I suppose you could _file_ the suit as a John Doe, but your real
name would have to be revealed before too many months, certainly
before it reached trial.
